Survived by his wife, Elaine Kubiak; a daughter, April (Richard) Bickart; 2 sons, Stephen (April) Kubiak and Edward Kubiak; a sister, Patricia Kozlowski; 5 grandsons, Matthew, Aaron, Jonathan, Tyler and Dylan. In lieu of flowers memorial donations may be made to Suncoast Hospice.
